The parent or adult student submitting this online form acknowledges our Policies as stated on the Arunodhaya's Liability Waiver and understands that his/her child or adult student is taking classes at his/her own risk and agrees to hold Arunodhaya Foundation, its instructors, and/or assignee harmless in the event of any physical injury to the student.

Person submitting this form is responsible for payment of tuition and dance uniform, recital and costume purchase fees. Please ensure that you have read and understood our Policies and signed the Liability Waiver before enrolling.
